Photo Release Form for Minors

I give Glitzy Glam Kids Mobile Spa & Lounge, LLC, permission to use my child’s likeness, image, voice, and/ or
appearances as such may be embodied in any pictures, photos, video recordings, digital images, and the like, taken
or made on behalf of Glitzy Glam Kids Mobile Spa & Lounge, LLC, party activities. I agree that Glitzy Glam Kids
Mobile Spa & Lounge, LLC, has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Glitzy Glam Kids Mobile Spa & Lounge, LLC business. These uses include, but are not limited to, reprints, publications, advertisements, social media, and any promotional materials in any medium now known or later developed, including the internet. I acknowledge that I will not receive any
compensation for the use of such pictures, etc., and hereby release Glitzy Glam Kids Mobile Spa & Lounge, LLC,
from all claims which are in any way connected with such use. I have read and understood this consent and
release.

Makeup Consent

As part of the Glitzy Glam Kids Mobile Spa & Lounge, LLC party experience age-appropriate makeup and spa
treatment products will be applied. Glitzy Glam parties are for entertainment purposes only. We exercise special
care in being sanitary and using mild kid-friendly products that are especially sensitive to young skin. Although, we
make no claims of being licensed professional technicians, cosmetologists, or estheticians. Therefore, we take no
responsibility for any allergic reactions of any party guest. It is the responsibility of the parent or guardian hosting
the party to alert Glitzy Glam in advance of any allergies that any of the children may have. Glitzy Glam reserves
the right to refuse services on any child, for any reason, that we feel may impose a health risk to others. I have read
and understood this consent and release.

Food Allergy

Your child will be participating in various activities on the Glitzy Glam Kids Mobile Spa & Lounge Bus. Additionally, some of the activities involve food products as part of the experience. Glitzy Glam takes every precaution to ensure that our girls are safe. Glitzy Glam Kids Mobile Spa & Lounge, LLC, will not be responsible for any allergic reactions or life-threatening incidents including death that could occur. If your child has any known food allergies that may prevent her from participating in these activities, we will provide an alternative option per your request. I have
read and understood this consent and release. By checking the box and booking a party with Glitzy Glam Kids
Mobile Spa & Lounge, LLC confirms I have read and understood this consent and release and assume all
responsibility as set herein.

Liability

We Strictly provide entertainment.  Please be aware that children need to be supervised.  We cannot be held
responsible for accidental damage caused by your child.  Glitzy Glam Kids Mobile Spa & Lounge, LLC, assumes no
responsibility or liability for accidents or damage caused during the party.  In the event of damage or loss or our
inventory and/or damage or loss to the party bus, the responsibility lies entirely on the client and or guardian. 
Glitzy Glam Kids Mobile Spa & Lounge, LLC, is not responsible for allergic reactions and/or any medical injuries
from the food, beverages, wearing costumes, or any activity taking place before, during, or after the party.
I hereby release and agree to hold Glitzy Glam Kids Mobile Spa & Lounge, LLC harmless from, and waive on behalf
of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages,
costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or
that may otherwise arise in any way in connection with any services received from Glitzy Glam Kids Mobile Spa & Lounge, LLC. I understand that this release discharges Glitzy Glam Kids Mobile Spa & Lounge, LLC from any liability
or claim that I, my family, or any personal representatives may have against the business with respect to any bodily
injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services
received from Glitzy Glam Kids Mobile Spa & Lounge, LLC. This liability waiver and release extends to the Glitzy
Glam Kids Mobile Spa & Lounge, LLC together with all owners, partners, and employees.

Coronavirus/COVID-19

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health
authorities still recommend practicing social distancing. I further acknowledge that Glam Kids Mobile Spa & Lounge, LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Glam Kids Mobile Spa & Lounge, LLC can not guarantee that I will not become infected
with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the
Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but
not limited to, Party staff, and other guest and their families. I voluntarily seek services provided by Glam Kids Mobile Spa & Lounge, LLC and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I and my family must comply with all set procedures to reduce the spread while attending a Glam Kids Mobile Spa & Lounge, LLC event.
